Outline Number Made Simple
Anytime you're working with paragraph-outline numbering schemes and some of your numbers start to misbehave, click on Format>Styles, click on the numbering style that's misbehaving, and delete it. The paragraph(s) where that style was applied will revert to Normal Style. Then you can set up the numbering style again and this time, it'll work.

Also, ALWAYS set up your outline numbering schemes at the first paragraph in the document where you need numbering applied. If you try to modify the numbering levels from anywhere else in the document except that very first numbered paragraph, you may soon find that some levels of your outline don't seem to know they're related to the others. In which case, refer to #1 above.
